RESEARCH TITLE: The effect of an osteopathic intervention on the intensity, frequency and on the quality of life in patients with migraine.
BACKGROUND: Migraine is a common chronic neurological disorder characterised by recurrent severe headache attacks, and causes a major disability with a huge socioeconomic burden. One in five patients treated with preventive medication will stop the treatment for tolerability and safety reasons. Osteopathic manipulative treatment (OMT), with the aim to restore the normal muscle tone and range of motion, can be helpful in reducing attacks. Patients with chronic headache may show peripheral and central sensitization, and a lack of habituation. OBJECTIVE: To evaluate the short-term effect of an individualized osteopathic treatment on the intensity, frequency and quality of life in patient with migraine. METHOD: Subjects were randomly assigned to an intervention or a control group. The intervention group received three individualized osteopathic treatments. There were two measurements, one before the first treatment and the second, two weeks after the last treatment. RESULTS: In this study (n=25), 13 patients (3 men, 10 women, age ± 36.7) received an osteopathic intervention and 12 patients (3 men, 9 women, age ±35.7) that received no treatment. The subject were asked to denote frequency of migraine attacks, the pain during migraine on NRS scale, to fill in the HDI questionnaire before the first and after the last treatment. At the intake there was no difference (Kolomogrov-Smirnov T-test p=0.22).  between the treatment group and control group. A power calculation was carried out beforehand and afterward, based on two studies. It was suggested to treat at least 12 patients in each group to arrive at a reasonable power. This study showed a power of 95.4%. The result show a statistically effect of the treatment on the NRS-scale (Wilcoxon T-test p=0.001), HDI (Wilcoxon T-test p= 0.01) and frequency (Wilcoxon T-test p=0.003) in the intervention in comparison with the control group: NRS-scale Wilcoxon T-test p=0.999), HDI (Wilcoxon T-test p=0.62) and the frequency  (Wilcoxon T-test p=0.999). The intervention group improves significantly compared to control group for all of the three measured variables: NRS-scale (Wilcoxon T-test p=0.001), HDI (Wilcoxon T-test p=5.64x10-5) and NRMIG (Wilcoxon T-test p=0.0008). DISCUSSION: In this study, the experimental group received a specific OMT depending on the individual needs in addition to conventional approach. This might not be ideal for research, but it reflects the reality in daily clinic. CONCLUSION: All though there was a good effect as a result of the treatment, further population-based studies with a more rigorous design are needed to provide stronger evidence supporting the effectiveness of the interventions.
