Abstract
ObjectivesTo investigate the effectiveness of a series of semi-standardised osteopathic treatments for patients who experience regular pain caused by primary dysmenorrhea. Design and SettingsAn open, prospective, randomised controlled trial comparing three groups: an osteopathic intervention group, a placebo-controlled sham group and a control group. ParticipantsFemales aged 18-40 years with regular menstrual cycles who experience regular dysmenorrhea with no known pathological causes. InterventionOsteopathic treatments were compared to three sham treatments or no treatment over the course of one menstrual cycle. The same researcher performed each semi-standardised intervention. Outcome measuresPrimary outcome measure was pain intensity assessed by Visual Analogue Scale (VAS). Secondary outcome measures were symptom duration and severity rated by the Cox Menstrual Symptom Scale (CMSS); quality of life assessed by Quality of Life Enjoyment and Satisfaction Questionnaire – Short Form (Q-LES-Q-SF); and menstrual blood loss assessed by Pictorial Blood Assessment Chart (PBAC). ResultsA total of 7 participants, average age 24.8, were randomised. There were no dropouts. A non-statistically significant reduction was found between baseline and post intervention for pain, symptom duration and severity, and quality of life for both osteopathic treatment and placebo-controlled sham groups, with no change for the control group. ConclusionsThis study was limited by a small sample size and poorly matched control group. No evidence was found to support osteopathy as a treatment for primary dysmenorrhea. Participants identified human touch/manual therapy as being helpful in the treatment of primary  dysmenorrhea. This study identified design changes that may improve future studies of primary dysmennorrheic pain.
