Abstract
This study aims to identify if there is an effect of hydrotherapy of the paravertebral muscles on pain pressure threshold (PPT). Twenty-one asymptomatic male students from British College of Osteopathic Medicine participated in a randomised single blind design study, for the duration of three weeks. The participants were randomly allocated to either the hydrotherapy (spinal douche at 36°Celsius), sham (modified ultrasound) or control groups, where treatment was administered once a week for 2.5 minutes on the paravertebral musculature. Using an algometer, PPT was recorded over the left paravertebral musculature at level of L5 spinous process (2cm away). The measurement was taken pre, post and post 24-hours after the intervention.A two way repeated measures ANOVA demonstrated there was a significant difference of time on PPT in the hydrotherapy group, with Week 1 Pre (W1PRE) showing an increased sensitivity versus Week 3 Post 24-hours (W3P24), (W1PRE=63.7N±13.9 Vs W3P24=95.0N± 15.0, p=<0.001). An increase in PPT and statistical significant difference was found in the hydrotherapy group each week pre versus post hydrotherapy for the duration of the study (W3POST Vs.W3PRE; p=<0.001), (W2POST Vs. W2PRE; p=<0.001), (W1POST Vs. W1PRE; p=0.027). There was no statistical significant difference between the groups; control and sham (p=0.358), intervention and sham (p=0.687) and intervention and control (p=0.702).There was no significant difference of time in either the control (p=1.000) or sham group (p= 0.998).The application of hydrotherapy demonstrated a hypoalgesic response, pre and post hydrotherapy each week, but does not extend to a post 24-hours hydrotherapy. A significant difference after three weeks of hydrotherapy in comparison to week 1 pre may indicate short-term effects of warm hydrotherapy on PPT of the paravertebral musculature. Further research is required to investigate for a significant difference between treatment groups, and additional research with symptomatic low back pain (LBP) subjects.
