Abstract
ObjectiveTo compare immediate and delayed effects of bilateral cervical (C1-C2) and lumbar (L1-L2) high velocity low amplitude thrust (HVLAT) technique on sub-occipital muscle pain pressure thresholds (PPT). DesignTwenty-four asymptomatic male subjects participated in a randomised control, single blinded study. Subjects were randomly assigned to cervical HVLAT, lumbar HVLAT, or control group. PPT measurements were taken pre, post, 1 day post, and 1 week post assigned intervention using an algometer on bilateral suboccipital muscles. A Two-way repeated measures ANOVA was conducted to identify PPT measurements between groups. Pearson product moment correlation coefficient was carried out to determine effects of age on PPT. Statistical significance was set at 0.05. ResultsLeft sub-occipital muscle PPT values showed no statistical significant difference in cervical and lumbar interventions throughout the study (P = 0.984). Right sub-occipital muscle PPT showed statistical significant increase in PPT 1 day post-intervention compared to pre-intervention (p=0.001). There was a statistical significant increase in PPT 1 week post intervention compared to pre-intervention (p=0.001). No statistically significant findings were found in PPT values for left suboccipital muscle during the study (p=0.984). No statistical significance was found for bilateral sub-occipital muscle PPT from lumbar HVLAT. ConclusionHVLAT of the upper cervical spine caused significant hypoalgesic effects in the right sub-occipital muscles 1 day and 1 week post-intervention. Further investigation into the long term effects of HVLAT should be conducted in a larger, symptomatic population.
