Abstract
Nutritional interventions are utilised by osteopathic practitioners in the treatment of some patients. With the increase in the market place of food supplements, many for the specialist athlete, claiming a variety of performance (athletic) and health benefits. Heavily advertised, with scientific test results printed on containers as evidence of efficacy, the author feels that there is a need to assess the validity of these claims.The aim of this study therefore is to conduct a systematic review of the literature in order to establish what research has been undertaken which evaluated the efficacy and safety of Leucine BCAA, and its metabolite, beta-hydroxy beta-methylbutyrate.For this purpose, a computerised literature search was performed using the Medicine database, followed by critical assessment of the methodologies used in selected studies. The main outcome measures were scores for methodological quality based on four broad categfories (study population, procedure reported, outcome, data analysis), subjective conclusion of authors and objective findings. Only six studies were identified which fulfilled the search criteria. The top scoring study scored 210 points with the second highest study only scoring 149 points out of a total 231 points, indicating the poor quality of the majority of studies. Five studies showed positive results, one study found negative results, as judged by their outcome measurements.The validity of using BCAA Leucine or HMB in supraphysiological dosages within a clinical environment is still unclear. To date there is minimal evidence to show that there is a positive effect in muscle structure and or function.
