Abstract
Introduction: Nowadays the disc herniations are a very frequent reason for consultation of osteopathy. Especially at the level L4-L5. It is usually accompanied by several symptoms that can be summarized as somatic dysfunction. The objective of this study is to analyze the relationship between the herniated disc of L4-L5 and somatic dysfunction.Methods: It is an observational study of type case and control, although in a transversal way. 30 people with 25-65 years old were analyzed with lower back pain. 15 had a herniated disc in L4-L5 (group cases) and 15 without a herniated disc (control group). The variables of somatic dysfunction (temperature change, asymmetry, restriction of movement and tissue change) and functional disability were investigated. Somatic dysfunction has been assessed through osteopathic exploration. And for the analysis of functional disability was used the Oswestry questionnaire.
Results: All 30 people had somatic dysfunction and functional disability. The variable tissue change has always been confirmed in both groups. The variables of temperature change (80% versus 46.7%) and movement restriction (80% versus 73.3%) were higher in group cases. In contrast, the variable asymmetry (60% versus 53.3%) was slightly increased in the control group. Regarding functional disability, in group cases was more dominant the moderate disability (66.7%) and in the control group the minimum disability (93,3%).
Conclusions: Definitely there is a relationship between the herniated disc L4-L5 and somatic dysfunction. Somatic dysfunction is accompanied by functional disability. The herniated disc aggravates this functional disability.
Abstract
Introducción: Las hernias discales son un motivo de consulta de osteopatía muy frecuente en la actualidad. Sobre todo en el nivel vertebral L4-L5. Suele acompañarse de varios síntomas que se pueden resumir como disfunción somática. El objetivo de este estudio es analizar la relación entre la hernia discal de L4-L5 y la disfunción somática.
Metodología: Es un estudio observacional del tipo casos y controles, aunque de forma transversal. Se analizaron 30 personas de 25-65 años con lumbalgia. 15 presentaban hernia discal en L4-L5 (grupo casos) y 15 sin hernia discal (grupo control). Se investigó las variables de disfunción somática (cambio de temperatura, asimetría, restricción de movimiento y cambio tisular) y la discapacidad funcional. La disfunción somática se valoró mediante la exploración osteopática. Y para el análisis de la discapacidad funcional se empleó el cuestionario Oswestry.
Resultados: Todos las 30 personas presentaron disfunción somática y discapacidad funcional. La variable cambio tisular siempre fue confirmada en ambos grupos. Las variables de cambio temperatura (80% frente 46,7%) y restricción de movimiento (80% frente 73,3%) eran más elevadas en grupo casos. En cambio la variable asimetría (60% frente 53,3%) era ligeramente aumentada en grupo control. Respecto a la discapacidad funcional, en grupo casos dominó la discapacidad moderada (66,7%) y en grupo control la discapacidad mínima (93%).
Conclusiones: Definitivamente existe relación entre la hernia discal L4-L5 y disfunción somática. La disfunción somática va acompañada de discapacidad funcional. La hernia discal agrava esa discapacidad funcional.
