Hintergrund: In der osteopathischen Literatur finden sich Anleitungen zur osteopathischen Behandlung des Glaukoms bzw. einer intraokularen Hypertonie als bedeutender Risikofaktor/bedeutendes Krankheitssymptom. Einige Studien konnten eine Abnahme des intraokularen Druckes durch eine osteopathische Behandlung nachweisen. Es existieren aber keine Untersuchungen der Validität der für Diagnose und Behandlung notwendigen Palpation des intraokularen Druckes (IOD) durch Osteopathen.
Ziele: Untersuchung der Validität der Palpation des IOD durch einen Osteopathen sowie Evaluation der Bedeutung der osteopathischen Untersuchung für die Glaukomfrüherkennung.
Methoden: Goldmann-Applanationstonometrie mit jeweils direkt anschließender Palpation des IOD an 74 einzelnen Augen bzw. bei 36 Augenpaaren im Seitenvergleich. Klassifikation der Ergebnisse gemäß der gängigen Bewertung in die Kategorien „hyperton“ (?22 mmHg) und „normoton“ (<22 mmHg) sowie „links höher/beide gleich/rechts höher“.
Results: Die positive LR für die Palpation des IOD beträgt 1,13, die negative LR 0,95. Weder für die Bewertung von „Normotonie/Hypertonie“ noch für die Bewertung „links höher/beide gleich/rechts höher“ konnte eine Übereinstimmung der durch das Goldmann-Applanationstonometer gemessenen Werte mit den durch die Palpation geschätzten Werten des Osteopathen festgestellt werden.
Diskussion: Zur Früherkennung des Glaukoms kann der Osteopath weniger durch seine Palpation als durch seine Beurteilung des gesamten klinischen Bildes des Patienten beitragen. Weitere Reliabilitäts-und Validitätsstudien mit erfahreneren Osteopathen und einer längeren vorausgehenden Trainingsphase sind nötig.

Title: Validity of the Palpation of the Intraocular Pressure by Osteopaths
Study design: methodological study in the field of fundamental research
Background: In the osteopathic literature guidance can be found regarding the treatment of glaucoma or intraocular hypertonia as considerable risk factor/ major symptom of the condition. Some studies were able to evidence a decrease of the intraocular pressure following an osteopathic treatment. However, there are no studies regarding the validity of the osteopaths’ palpation of the intraocular pressure (IOP) necessary for diagnosis and treatment.
Objectives: To study the validity of the palpation of the IOP by an osteopath and assess the relevance of an osteopathic examination regarding the early detection of glaucoma.
Methods: Goldmann applanation tonometry with immediately subsequent palpation by the blinded osteopath. The IOP of 74 individual eyes was examined and 36 pairs of eyes used for bilateral comparison. Classification of the results according to the well-established categories “hypertonic” (?22 mmHg) and “normotonic” (<22 mmHg) as well as “higher left/ both equal/ higher right”.
Results: The positive likelihood ratio (LR) for the palpation of the IOP is 1.13, while the negative LR is 0.95. Neither for the classification of “normotonia/ hypertonia” nor for the classification “higher left/ both equal/ higher right” a correlation between the values estimated by the osteopath and the values measured by the Goldmann applanation tonometry could be observed.
Discussion: Regarding an early detection of glaucoma osteopaths can contribute more with their overall evaluation of the patient’s clinical picture than with their palpation. Further reliability and validity studies involving experienced osteopaths and a preliminary extended training period are necessary.
