Abstract
Introduction: Total Body Adjustment (TBA) is assumed to have positive
effects on the lymphatic system according to classical osteopathy. TBA was
therefore applied on patients suffering from upper extremity stage II
lymphedema after breast càncer. An osteopathic approach was considered
since the problema is freqüent and important. The objective was to analyze
volumetric and funcional changes.
Methodology: randomized pilot study, with a sample group of twelve women
(age 50-65) with stage II lymphedema. Treatment groups: treatment (TBA
routine) and control (placebo routine). The two groups were treated once a
week during a month. DASH and perimeter measures of the upper extremity
were recorded during three evaluations: pre-treatment, post-treatment and
one month after last treatment.
Results: significant improvement was seen post-treatment (p <0.05). The
third evaluation, one month after last treatment, contained significant
changes in: styloids (p = 0.01), distal epicondyle (p = 0.019) and proximal
epicondyle (p = 0.001).
Conclusions: TBA significantly reduces upper extremity lymphedema and
improve the short-term quality of life. Improvement is also seen in three of the
distal measures, but not in MTCF and DASH.
Abstract
Introducció: segons l’osteopatia clàssica el TBA té efectes beneficiosos en
el sistema limfàtic. S’aplicà TBA en pacients amb limfedema d’extremitat
superior estadi II post càncer mamari. Es plantejà un abordatge osteopàtic
donat que és un problema força prevalent i important. L’objectiu fou analitzar
els canvis volumètric i funcionals.
Metodologia: estudi pilot aleatoritzat. Grups: d’estudi (rutina TBA),
control (rutina placebo). Mostra de 12 dones (50-65 anys) amb limfedema
benigne, unilateral d’extremitat superior, en estadi II i secundari al càncer de
mama. Es realitzà un tractament setmanal durant un mes. Es calculà el
DASH i mesures perimètriques d’extremitat superior en tres avaluacions:
pre-tractament, post-tractaments i un mes després de l’últim tractament.
Resultats: millora significativa en la segona avaluació (p<0,05). En la
tercera avaluació es mantenien canvis significatius en tres mesures:
estiloides (p=0,01), distal epicòndil (p=0,019) i proximal epicòndil (p=0,001).
Conclusions: el TBA redueix significativament el limfedema d’extremitat
superior i augmenta la qualitat de vida a curt termini. La millora es manté en
les tres mesures més distals i no en la MCF ni en el DASH.
