Studiendesign: Systematisches Review auf der Basis
von RCT-Studien, welche anhand der Cochrane risk-of-bias
tool for randomized trials Version 2 (RoB2) beurteilt
werden.    

Methode:
Als Grundlage dienen randomisierte kontrollierte Studien, die sich mit der
therapeutischen Wirksamkeit der Osteopathie bei gesunden und kranken
Erwachsenen auf die Atemfunktion auseinandersetzen. Dabei wurden die Datenbanken Pubmed, Google scholar,
Cochrane liberary, The Journal of the American Osteopathic Association (JAOA),
International Journal of osteopathic medicine (IJOM), Journal of body work and
movement (IMBT), Osteopathic medicine and primary care journal (OM-PC),
Osteopathic research durchsucht. Das Verzerrungsrisiko und die Wirkung
wurden analysiert. Die Studienmerkmal und die Ergebnisse daraus wurden
zusammengefasst.

Ergebnisse: Insgesamt wurden 10 Studien nach
Setting, Population, OMT-Techniken, Behandlungsparameter, Qualität der
Datenermittlung, Übereinstimmung der Hypothesen und Outcomes sowie nach dem
Verzerrungsrisiko analysiert. Die primären Techniken waren Doming the
Diaphragma, Suboccipitale Dekompression, RR, TLP. Behandlungen mit einer
Kombination von OMT-Techniken und über einen längeren Zeitraum hinweg erzielten
positive Veränderungen bei gesunden und kranken Erwachsenen. Die Messdaten
wurden primär mit Spirometrie, Plethysmographie, Ultraschall, Blutbild und
statischem Test (CAT) ermittelt. In fünf Studien wurden die Hypothesen
bestätigt. In
allen fünf Domänen überwiegt das low risk of bias, es kam insgesamt 41 Mal vor.
Sechs Mal wurde some concerns und drei Mal high risk of bias registriert. Das
Ergebnis des Gesamtrisikos kann insgesamt als low of risk of bias bezeichnet
werden.

Diskussion: Die OMT-Behandlung als primäre
Therapie oder kombiniert mit SPR beeinflussen positiv die Reduzierung der Tage
der rein oralen und oral-intravenösen Antibiotikabehandlung, die Länge des
Klinikaufenthaltes und die Lungenfunktionsparameter.

Study
Design: Systematic Review based on RCT studies,
which are assessed using the Cochrane risk-of-bias tool for randomized trials
version 2 (RoB2).

Method: Randomized controlled trials were used as a basis to examine the
therapeutic efficacy of osteopathy on respiratory function in healthy and sick
adults. Eight medical and
osteopathic databases were searched. A total of 10 studies were analysed
according to setting, population, OMT techniques, treatment parameters, quality
of data collection, consistency of hypotheses and outcomes as well as the risk
of bias.

Results:
The primary techniques used were doming the
diaphragm, suboccipital decompression and rib raising. Treatments using a
combination of OMT techniques carried out over a longer period of time achieved
positive changes both in healthy and sick adults. The measurement data were
determined primarily with spirometry, plethysmography and ultrasound. In five
studies the hypothesis was confirmed. Concerning the risk of distortion, three
studies had a high risk of bias, six studies had a low risk of bias, and one
study was assessed with some concern.

Discussion:
OMT treatment as primary or complementary therapy
positively affects the reduction of days of pure oral and oral intravenous
antibiotic treatment, the length of hospital stays and the lung function
parameters.

Conclusion: Future studies should evaluate and investigate the effects of
individual osteopathic techniques and variants of techniques on the respiratory
system.
