Abstract
Background Although there is evidence to suggest that un-delayed operative treatment has the best outcomes for ACL reconstruction in the skeletally immature there is contention regarding the different outcomes of the different surgical techniques, the tissue type and form of graft used. Objective To investigate the relationship between the different surgical techniques, the type and form of graft used and the outcomes of these techniques in the skeletally immature population. Design Structured literature review. Method A computerised search was conducted using Boolean operators in the following databases; PubMed, ScienceDirect, Swetwise and GoogleScholar. Articles were then included or excluded based on the following criteria; Inclusion * Female (<18) or male (<21) human subjects, * Specifies reconstruction technique, * Specifies graft, * Specifies age of subjects at time of surgery and/or gives evidence of physeal patency using imaging or Tanner scale, * ACL mid substance tear, with or without meniscal injury, * Published 2004 – 2014, * Includes data on follow-up time,* Includes data on total number knees/subjects. Exclusion * Subjects with closed physes, * Non-English language studies, * Posterior cruciate ligament, *Lateral or medial collateral ligament,* Non-operative or studies including non-operative subjects/cohorts, * Unable to access the data from journal articles or abstracts freely, *ACL repair, * Avulsion facture of the tibia or femur. The included articles were then analysed and the relevant data was captured. Results A total of 744 articles were found on the initial search of the databases and 25 articles met the inclusion/exclusion criteria. Discussion The results were inconclusive however methodological issues found within the articles include and the outcome measures used were explored. Conclusion The results of this systematic literature review were inconclusive because there is little or no correlation between the type of surgical technique, form of graft used and outcomes. The results suggest that the knowledge base for the management of ACL injuries in the skeletally immature is weak. We would therefore advise that further high quality research be carried out in this field.
