Abstract
Spinal palpatory tests of the lumbar spine: an interobserver reliability study among students of year 4. Objective To investigate the interobserver reliability of 4th year osteopathic students in their assessment of lumbar somatic dysfunction using four selected osteopathic palpatory diagnostic tests. Methods Four blinded examiners (4th year students at the European School of Osteopathy) assessed the lumbar region of a sample of ten asymptomatic subjects using the following palpatory tests: asymmetry, tissue texture changes, motion and tenderness. Prior to the assessment the examiners attended a theoretical/practical pre-training in order to get familiar with the assessment procedures. Also, a pre-assessment session was conducted where examiners paired and agreed on the marking of the spinous processes. The data were analysed using Agreement Coefficient I (AC I ) and Cohen's Kappa and reported with a 95% confidence interval (CI). The qualitative scale used to benchmark the results was the Landis and Koch scale. ACI was interpreted using the Gwet benchmarking method.Results Estimated ACI value for the tenderness test was interpreted as _fair' (ACI=0.499; ACI and Kappa Asymmetry, tissue texture changes and motion test results for both ACI (-0.078+ 0.015) and Kappa (-0.105 -0.015) were interpreted as _poor' and below 0.4 so were considered clinically unreliable. Conclusion Fourth year students of ESO demonstrated a clinical acceptable interobserver reliability for the tenderness test. Inter-rater reliability for asymmetry, tissue texture changes and motion tests were consistently interpreted as _poor' and clinically not
