Abstract
OBJETIVES: To evaluate by comparison the results obtained from the study of the comprehensive range of active movements in the cervical spine and the pain threshold between the technique of manipulation with impulse (thrust) in rotation (TMR) and a placebo technique (TP) of the vertebral segment C3-C4.

HYPOTHESIS: In patients with mechanical neck pain, the TMR applied to the zygapofisary joint with dysfunction of the vertebral segment C3-C4, produce an immediate increase in the pain threshold and the comprehensive range of active movements in the cervical spine.

MATERIAL AND METHODS: A randomized controlled experimental trial, simple blind. Assessments were done before/after intervention of the comprehensive range of active movements in the cervical spine and the pain threshold, in patients with pain and restriction of lateral displacement of joint segment C3-C4. In total seventy (70) patients, chosen at random, have participated; a group of 35 patients were treated by means of TMR and a control group of 35 patients by means of TP. We used a cervical goniometric (CROM) and a visual analogue scale (VAS) for assessments.

RESULTS: The within group changes showed a significant improvement in neck pain at rest and mobility after application of the TMR (p<0.0001). The control group also showed a significant improvement in neck pain at rest (p<0.04), extension (p<0.004), the flexion (p<0.01) and both lateral flexion (p<0.01, p<0.03), but not in rotation. The greatest increase in neck mobility was negatively associated to the decreased neck pain.

CONCLUSIONS: The TMR applied to the joint segment C3-C4 augments immediately the full range of active movements of the cervical spine, as well as the pain threshold.
Abstract
OBJETIVOS.- Valorar de forma comparativa los resultados obtenidos en la amplitud global de los movimientos activos del raquis cervical y umbral del dolor entre la técnica de manipulación con impulso (thrust) en rotación (TMR) y una técnica placebo (TP) aplicada en el segmento vertebral C3-C4.

HIPOTESIS.- En pacientes con cervicalgia mecánica crónica, la TMR aplicada en la articulación interapofisaria en disfunción del segmento vertebral C3-C4, produce un aumento inmediato en el umbral del dolor y en la amplitud global de los movimientos activos de la columna cervical.

MATERIAL Y MÉTODOS.- Estudio experimental controlado aleatorizado, simple ciego. Se han realizado mediciones pre/post-intervención de la amplitud global de los movimientos activos de la columna cervical y del umbral del dolor, en pacientes con dolor y restricción al deslizamiento lateral del segmento articular C3-C4. Han participado un total de setenta (70) pacientes asignados aleatoriamente en un grupo de intervención mediante TMR de 35 pacientes y en un grupo control mediante TP de 35 pacientes. Utilizamos un goniómetro cervical (CROM) y la escala visual analógica (EVA) para la toma de mediciones.

RESULTADOS: Los resultados obtenidos mostraron un aumento significativo en el dolor cervical y la movilidad después de aplicar la TMR (p<0.0001). El grupo control también mostró aumentos significativos en la disminución del dolor (p<0.04), la extensión (p<0.004), la flexión (p<0.01), ambas lateroflexiones (p<0.01, p<0.03) pero no en rotación. El aumento del movimiento cervical fue negativamente asociado al descenso del dolor cervical.

CONCLUSIONES.- La TMR aplicada sobre el segmento articular C3-C4 aumenta inmediatamente la amplitud de los movimientos activos de la columna cervical, así como el umbral del dolor.
